The Personal Health Information Protection Act, 2004
(PHIPA) is an Ontario law that governs the collection, use and disclosure of
personal health information within the health sector. The object is to keep
personal health information confidential and secure, while allowing for the
effective delivery of health care.)
Requesting Access to
Personal Health Information
Under the Act, you have the right to access and request a
copy of your own personal health information. If you like to access, please:
- Refer your request to the Release of
Information office in Health Records (located level 1). The Request Consent for Release Disclosure of Patient Health Information must be completed as a
written request to access their information and also serve as a record of
who accessed the record.
- Please note the request fees are subject to
each request made. The fee schedule can be found on the back of the
- CHEO has up to
30 days to process a request in accordance with the PHIPA.
How To Access Your
Personal Health Information:
You can obtain a copy of, or view your health record, by
contacting CHEO’s Health Record Services. They can be reached by phone at
613-737-7600, ext. 2292, or by fax at 613-738-4855. Please note, we require a written request that is signed by the patient (if 12 years of age or older) or the parent/legal guardian before the request can be fulfilled.
Information Office hours are Monday – Friday: 8am – 4pm,
excluding stat holidays.
We require the following to be included as part
of the authorization:
must be directed to CHEO
include patient’s name, date of birth and medical record number if known

include the name, address and the phone number of the requester
authorization must be fully completed, dated and signed
